PAYMENT CONTRACT – MAY 27, 2008 – AUGUST 8, 2008

 

Note: Please complete one contract for each child who will attend.

The following conditions involved in the care of ________________________________ are understood and agreed upon between CHBC Summer/Afterschool Program and ____________________________________.

C. Contract Fee Schedules: ($50 discount if entire amount paid by May 27 – any plan.)

¨ PLAN A – Full-Time Rate (will be billed for the entire eleven weeks – takes into account a two-week vacation)

     $95 per week (first child) -- $1045

     $90 per week (each additional sibling) -- $990

 

¨ PLAN B1 – Part-Time Rate – Any 33 days over the 11 week summer term

     (1/3 of total due on May 27, 1/3 of total due on June 23, 1/3 of total due on July 28)

     $875 for summer (first child)

     $770 for summer (each additional sibling)

 

¨ PLAN B2 – At least 20 specifically contracted days during the summer term

     (1/2 of total due on May 27, 1/2 of total due on June 30

     $27 per day (first child) – Minimum $540 summer cost

     $26 per day (each additional sibling) – Minimum $520 summer cost

 

¨       PLAN C – Drop-in

      (payment due day of attendance)

     $35 per day per child

 

These fees are to be paid at the time of application:

Transportation fee $50 per child (Drop In pays $5.00 transportation per swimming/field trip day.)

 

Registration Fee: $100.00/family -- $80.00 will be credited toward final payment for the summer provided account is current.

I (We) understand that I (we) are contracting to pay a total of $________ for the 2008 Summer/AfterSchool Program. Any days in attendance beyond those contracted for will be assessed at the daily rate. Payment for additional days is required prior to or on the day of service.

 

The Program agrees that:

In return for the sum which the parent agrees to pay, the Program will give regular care to the above-named child from 7:30 AM – 6:00 PM, May 27 – August 8, 2008.

If your child is absent due to illness or death in the immediate family for more than three (3) consecutive days, the rate will become half your contracted rate for the remaining contracted days missed. This request must be made in writing to the Advisory Committee before the provision can be implemented.

 

The Parent/Guardian agrees:

1)    to pay the program for contracted days on or before service, according to the schedule below. The first payment must be made no later than the first day of care and all payments must be made in full by July 28, 2008. Any other payment arrangements must be requested in writing and approved by the Program’s Advisory Board. Children will not be allowed to attend if accounts are over 5 days past due.

2)    to stay with this contracted agreement payment throughout the summer. Changes may be made under only very unusual circumstances per written request to and approval by the Program’s Advisory Board.

3)    that the Program may withdraw care for the child(ren) if any account becomes five days past due.

4)    to pay a $25.00 bookkeeping fee for any account balance which becomes past due and/or for any additional days not paid for on or before the day of service. Late fees are applied the day following the due date. Returned checks will be charged a $25.00 return fee and must be repaid in cash.

5)    to pay any fees associated with the collection of past due balances plus the full amount owed plus any late fees if my account must be referred to a collection service or small claims court.

6)    to pick up my child by 6:00 PM or be charged a late pick-up fee of $10.00 for the first 15 minutes and an additional $1.00 for every minute thereafter. The late pick up fee must be paid at the time the hcild is picked up or a $25.00 bookkeeping fee will be charged.

7)    that the parent(s)/guardian(s) signed below are responsible and liable for timely payment of all fees due.

8)    to provide the Program with all properly completed and signed forms and papers required for the Program to provide care for my child.

9)    to provide the Program any medical, emotional, education, physical or other information that will enable the Program to provide adequate and safe care for my child and for the other children and staff participating in the Program.

10) to pay select field trip fees, with advance notice from Program.

 

Payment Schedule: Payments must be made as follows:

1)    PLAN A – Bi-weekly, beginning May 27, 2007

3)    PLAN B1 – paid fully in three installments, 1/3 on May 27, 1/3 on June 23 and 1/3 on July 28

4)    PLAN B2 (contracted days) – paid fully in two installments, 1/2 on May 29 and 1/2 on June 30

5)    PLAN C (drop-in) – paid on each date of service

6)    ALL FEES MUST BE PAID IN FULL BY July 28, 2008.

 

Failure to keep payments current can result in termination of child care until balance is paid. Such termination will not release you from the contract.
